Claim Forms

To notify us about a claim, please use our PDF forms in which all necessary information are requested. You can send your completed form directly to our claims department, email: szkody@pantaenius.pl. Please note that we always need a version with your signature to process your claim. You can send it to us scanned by e-mail or fax or by traditional mail.

Instructions for editing on a PC: Save the document on your PC before starting to fill in the data. Make sure that an updated version of Adobe Acrobat Reader is installed on your PC.

Instructions for editing on a Tablet or Smartphone: Install the latest version of Adobe Acrobat Reader app on your device and use it to open the document.
